Terms of Service

These Terms of Service ("Terms") govern your access to and use of the website at fieldvitality.com (the "Site") and any related services (collectively, the "Services") offered by Floreo Vitality LLC, a Wyoming limited liability company doing business as Field Vitality ("Field," "we," "us," or "our"). By accessing or using the Services, you agree to be bound by these Terms.

1. Who we are

Field is a consumer wellness brand. We connect adult consumers in the United States to a network of independent, licensed physicians who provide telehealth consultations, and to an independent LegitScript-certified compounding pharmacy that may dispense and ship prescription products. Field does not practice medicine, dispense prescription products, manufacture compounded products, or hold pharmacy licenses. All clinical decisions are made by independent licensed physicians; all dispensing is performed by the LegitScript-certified compounding pharmacy.

2. Eligibility

You must be at least 18 years of age and a U.S. resident to use the Services. By using the Services you represent that you meet these requirements.

3. Telehealth consultations

If you initiate a consultation through the Site, you are entering into a patient-provider relationship with an independent licensed physician in the field provider network, not with Field. The physician will evaluate your intake and determine whether a prescription product is appropriate for you. A prescription is not guaranteed.

4. Products and fulfillment

If a physician prescribes a product, the prescription is sent to a LegitScript-certified compounding pharmacy that dispenses and ships the product directly to you. Field receives a brand and customer-experience fee; Field does not handle, store, or ship prescription products at any point.

5. Subscriptions and billing

Many programs are sold as monthly subscriptions. By subscribing, you authorize Field to charge your payment method on a recurring basis until you cancel. Cancellation is available anytime via the patient portal, email, or phone, effective for the next billing cycle. See our Refund and Cancellation Policy for details.

6. Disclaimers

The Services are not a substitute for emergency medical care. If you are experiencing a medical emergency, call 911 or your local emergency number. Information presented on the Site is not medical advice and does not establish a patient-provider relationship.

7. Limitation of liability

To the maximum extent permitted by law, Field, its officers, members, employees, and contractors shall not be liable for any indirect, incidental, consequential, special, or exemplary damages arising from your use of the Services.

8. Governing law and dispute resolution

These Terms are governed by the laws of the State of Wyoming. Any dispute arising out of or relating to these Terms shall be resolved by binding arbitration administered by JAMS in Cheyenne, Wyoming, pursuant to its Comprehensive Arbitration Rules.

9. Changes

We may update these Terms. Material changes will be posted on this page and become effective on the date noted at the top.
