This Privacy Policy describes how Floreo Vitality LLC ("Field") collects, uses, and protects information when you visit fieldvitality.com or interact with our Services.

1. Information we collect

• Information you provide: name, email, phone, intake responses, payment information, and any communications you send us.
• Health information: information you submit in the intake flow is transmitted to an independent licensed physician and to the LegitScript-certified compounding pharmacy for clinical purposes. Such information is handled under applicable health-information laws by the physician and pharmacy.
• Usage information: standard web analytics, including IP address, browser type, pages viewed, and referrer.

2. How we use information

• To provide the Services, including routing your intake to a physician and confirming consultations
• To process payments and manage subscriptions
• To respond to support inquiries
• To improve the Site and Services
• To comply with legal obligations

3. Sharing

We share information with: (a) the independent licensed physician you consult through the field network; (b) the LegitScript-certified compounding pharmacy that dispenses any prescribed products; (c) our payment processor; (d) service providers acting on our behalf; and (e) as required by law. We do not sell personal information.

4. Security

We use commercially reasonable administrative, technical, and physical safeguards to protect information. No method of transmission over the internet is completely secure.

5. Your choices

You may request access to, correction of, or deletion of your account information by emailing support@fieldvitality.com. Health-information requests directed to the prescribing physician or pharmacy should be made to those entities directly.

6. State-specific rights

Residents of California, Virginia, Colorado, Connecticut, Utah, and other states with comprehensive privacy laws have additional rights. To exercise these rights, email support@fieldvitality.com.

7. Children

The Services are not directed to children under 18. We do not knowingly collect information from children.

8. Changes

We may update this Privacy Policy. Material changes will be posted with a new effective date at the top.
